Not every note or letter starts with a consultation. With the Create button, you can start a note or letter at any time without recording a conversation first. For example, you can dictate a letter after clinic, or turn information you already have into a structured note.

Start a note or letter without recording a consultation.
Choose from your workspace’s note and document templates.
Dictate straight into it, or generate it from context you add, such as an existing referral.
Continue it as a full consultation later if you need to add more.
📝 How to start a note or letter with the Create flow
From the Scribe home screen, click ‘Create’. You don’t need a patient list open or a consultation in progress
Choose ‘Note’ or ‘Document’ from the filter on the left, or browse ‘All’.
Select the template you want to use.
Choose how to fill it in: dictate directly into it, the same way you would during a consultation, or generate it from context you’ve added, such as an existing referral or document.
Review and edit it as usual, by typing, dictating or using ‘Adjust’.
💡 Generating from context is useful when the information you need already exists in another document, and you just want Scribe to structure it for you.
⚠️ It is essential that the clinician reviews all of their Accurx Scribe outputs before they are added to the medical record or shared with patients and other services.
▶️ Continuing as a full consultation
If you need to add more once you’re with the patient, click ‘Resume’ at the bottom to pick up live recording. Your note or letter then carries on as a full consultation.
If an encounter has more than one note or letter, each one appears as its own tab (for example, Note 1 and Note 2), so you can switch between them at any point.
📤 Saving and sending
Once your note or letter is ready, save and send it in the same way as any other Scribe output. For more, see:
