Accurx Scribe in Web: How to do a consultation

Setting up with Accurx Scribe powered by Tandem and how to run your first consultation

We've outlined below the steps for using Accurx Scribe from Accurx Web.

1. Open up Scribe from the top of your Accurx Web homepage

2. Start the consultation

You can easily start the consultation by clicking on "Start" in the middle of the screen.

Consent prompt: If your clinic has turned on the consent prompt setting, a pop-up will appear when you click "Start". Confirm that you’ve asked the patient for consent, and the Scribe will start as normal.

This setting is off by default and can only be turned on or off by a clinic admin, under clinic settings — it applies to everyone in the clinic. The prompt is a reminder only: it doesn’t record the patient’s response or keep a log of confirmations, so continue to document consent in the note in line with your organisation’s local policy.

Once this setting is toggled the Patient consent pop-up will appear each time you Scribe 👇

3. Conduct the Patient Call or Dictation

You can meet with the patient as usual, while the Scribe runs in the background. You can also dictate directly into the Scribe by speaking naturally, and it will generate a complete medical note based on what you say. Accurx Scribe's AI automatically understands what information should be written under which section and how to structure the note.

  1. Appropriate headings: Ensure your template includes all the information you want to document.

  2. Clear instructions: Provide Tandem with precise keywords and concise summaries.

4. Create the Note

Click on "Finish and create note" when you’re done, and the Scribe will generate the note based on the conversation you just had.

Templates: If you have multiple templates, you can choose which template you’d like Accurx Scribe to structure your note in. This can also be done in the next step.

Resume encounter: Select "Resume Encounter if you need to resume conducting or transcribing a Scribe consultation note that has already been finished.

Add additional text: Click "Add entry" or "Adjust" to add information, such as previous notes or a digital patient record. This will be used alongside the recording to create the medical note. You can also create a note solely based on text input, without any recording.

Tandem includes a built-in feature to automatically add more to your note or adjust your note based on your instructions using AI. Click the 'Adjust' button at the bottom of your Scribe note to open this feature.

If you only need to adjust specific parts of your Scribe note, click on the individual sections you want to change. A blue border will appear around them, and your selections will be pinned as badges above the text input. Your typed or verbalised instructions will then only apply to those selected sections.

If you don't select specific parts of your Scribe note to adjust, the typed or verbalised instructions will be applied to the whole Scribe note.

Copying your note and marking it as transferred

When your note is ready, click 'Copy' to place the content on your clipboard, ready to paste into your clinical system.

Copying doesn't change your note's status. Selecting 'Copy' only copies the content — it won't mark the note as transferred. Marking a note as transferred is a separate, deliberate step, so you stay in control of when it happens.

To mark a note as transferred, after copying, click the chevron (˅) next to the primary button and select the option to mark it as transferred.

Transferred status applies to each note or letter individually, not to the whole consultation. If a consultation includes both a note and a letter, each has its own status, and they can sit in different states — for example, a note marked as transferred while its letter is not.

You can check the status of each note or letter from the consultation header or from the sidebar, where the status is shown for each item individually.

Speaker Diarisation

Scribe can now tell different speakers apart, so multi-person consultations, ward rounds, and MDTs come through more clearly. Cleaner transcripts when more than one voice is in the room.


It is essential that the clinician reviews all of their Accurx Scribe outputs before they are added to the medical record or shared with patients and other services.

Reporting these helps our team review the output and continue improving the tool's accuracy.

  • Hallucinations: Any false or misleading information that was added.

  • Omissions: Any relevant clinical information that was missed.

5. Create a document from the Scribe notes

Accurx Scribe also has a feature that uses your consultations to create follow-up documents, including patient, referral, and outpatient letters. With just one click, Scribe handles the drafting for you, saving you from the manual paperwork input 🎉

💡 Pro-tip: Managing your daily usage

To ensure your consultations are processed correctly and to manage your daily usage effectively, keep the following in mind:

  • Finalise promptly: Always click Finish and create note as soon as an encounter ends. A session counts toward your daily usage if it remains "active". This includes any sessions that are left in a Paused state or are open without an end timestamp.

  • Testing Scribe: If you are just testing Scribe, keep your recording under 2 minutes and remember to click Finish and create note to ensure it doesn't count as a full session.

  • Dictation: Note that using Dictation mode is also counted as an active session.
