What is Patient Triage?

Accurx Web’s Patient Triage solution uses an online, bespoke form to give patients a clear, structured way to contact your service about their care, while providing clinical teams with the information they need to prioritise and respond appropriately.

Instead of relying on phone calls, inboxes, or automatically booked follow-ups, Patient Triage supports a triage-first model, where requests are reviewed by the team and action is taken based on clinical need. See a demo Patient Triage form here.

When is Patient Triage useful?

1️⃣ Patient-Initiated Follow-Up (PIFU) pathways

Patient Triage works well for services using PIFU, where patients don’t attend routine follow-ups unless they need to.

Patients can contact the service if:

  • Their symptoms change or worsen

  • They have questions about treatment or medication

  • They need help with results or admin queries

The team reviews the request and decides whether it can be resolved digitally, requires further review, or requires an appointment.

2️⃣ Ongoing access for long-term or specialist care

For services supporting patients with long-term or complex conditions (for example, CNS-led helplines), Patient Triage provides a consistent way for patients to raise concerns between appointments.

This helps teams:

  • Intervene earlier when needed

  • Prioritise urgent requests more safely

  • Reduce pressure from phone calls and unmanaged inboxes

3️⃣ Remote monitoring and pathway-based care

For patients on a monitoring or surveillance pathway, Patient Triage serves as a safety net.

Patients can report new symptoms, concerns, or admin needs, and the team can decide what action is needed without bringing patients in unnecessarily.

4️⃣ Between booked appointments

Even when patients already have follow-ups booked, things change.

Patient Triage allows patients to get in touch in a structured way if:

  • Their symptoms improve or worsen

  • They have questions that don’t need an appointment

  • New information might change the need or urgency of their visit

This helps ensure appointments are used where they add the most value.

Why use Accurx Patient Triage?

Patient Triage supports a calmer, more sustainable way of working:

  • Requests are reviewed at a time that works for the team

  • Clinical judgement stays with your service

  • Many issues are resolved without appointments

  • Appointments are reserved for patients who really need them

This improves patient experience while helping teams manage demand more safely.
