Accurx Scribe in Desktop: How to do a consultation

Setting up with Accurx Scribe powered by Tandem and how to run your first call

Powered by Tandem, Accurx Scribe is your AI solution that:

  • cuts your admin time

  • removes the typing so you can focus more on your patients

  • connects care by working seamlessly with your Accurx toolbar.

1. Open Accurx Scribe from your Toolbar

If this is your first time using our new Scribe powered by Tandem you will need to:

  • Ensure you are an approved member of your organisation

  • Confirm all relevant governance and compliance steps have been completed (e.g. DCB0160 and DPIA)

  • Accept our new DPA and Terms and Conditions

2. Obtain patient consent

It is best practice to always obtain patient consent for the use of AI transcription and summarisation before starting the Scribe consultation. You can document this in the Scribe note summary. Dot phrases allow you to do this in 2 clicks.

3. Start the consultation

You can easily start the consultation by clicking on "Start" in the middle of the screen.

4. Conduct the consultation or dictation

You can now meet with the patient as usual, while the Scribe runs in the background. You can also dictate directly into the Scribe by speaking naturally, and it will generate a complete medical note based on what you say. Accurx Scribe's AI automatically understands what information should be written under which section and how to structure the note.

  1. Good audio: Use a reliable conference microphone.

  2. Appropriate headings: Ensure your template includes all the information you want to document.

  3. Clear instructions: Provide Tandem with precise keywords and concise summaries.

5. Create the note

Click on "Finish and create note" when you’re done, and the Scribe will generate the note based on the conversation you just had.

Templates: If you have multiple templates, you can choose which template you’d like Accurx Scribe to structure your note in. This can also be done in the next step.

Add additional text: Click "Add additional text" to add information, such as previous notes or a digital patient records. This will be used alongside the recording to create the medical note. You can also create a note solely based on text input, without any recording.


6. Review the note

Accurx Scribe is powered by Tandem's AI. Whilst it's highly accurate, it can make mistakes, such as missing important information out (omissions) and confabulating incorrect information (hallucinations).

It's very important that the responsible clinician reviews, and edits if necessary, all the Accurx Scribe outputs before adding them to the medical record or sharing documents with patients or other services.

7. Save the note to your medical record system or copy it

After reviewing the note and making any necessary edits, click the button in the bottom right to save it to your medical record system or to copy it. It is recommended that notes are saved or copied to the medical record as soon as possible after completing a consultation.

The EMIS and SystmOne APIs only allow Accurx Scribe to save notes as new record entries, not into open consultations, so they may be in a different format to how they would appear if manually entered by the user. This could result in clinical information being missed, or the note not being linked to relevant coding, prescriptions, referrals, investigations, or appointments.

If needed, you should ensure that you edit your notes in the medical record to add additional information, coding (e.g. problem codes) and to set the consultation type.

EMIS users can do this by editing the Scribe entry in the medical record. SystmOne users can do this by editing elements of the Scribe entry in the medical record (e.g. associate with problem codes, edit the text) or by adding additional coding, prescriptions and referrals as new note entries.

SystmOne users should also ensure they click 'Save' in the medical record to avoid discarding their Scribe note entry.
