Accurx Scribe: Encounter linking

How to connect a previous consultation so Scribe can use its notes and letters as context

Written by Helen Merriott

Encounter linking lets you connect a patient's previous consultations to the one you're doing now. Scribe then uses the connected notes and letters as context when it writes your note or letter.

It's useful for follow-ups, where you'd usually work from the last clinic letter. Diagnoses and medications from the previous letter can carry through, so you don't have to dictate them again.

ⓘ Availability: Encounter linking is available to select organisations. If you can't see it, speak to your Accurx account manager.

How to connect a previous encounter

1. Open the Context tab

Start your Scribe consultation for the patient, then open the Context tab and click Connect encounter. You'll see a list of your previous consultations.

2. Choose a previous consultation

Select the consultation you want. A preview opens, with a separate tab for each note and document from that consultation.

3. Connect the notes or letters you want to use

Connect one or more notes or letters. A tick appears on each one you've connected, and they'll be listed on the Context tab.

You can connect more than one consultation.

⚠️ Check it's the right patient: Scribe doesn't check that a connected consultation belongs to the same patient.

4. Disconnect anything you don't need

To remove a connected note or letter, disconnect it. Click the three dots, then 'disconnect'.

How Scribe uses connected encounters

When you generate a note or letter, Scribe uses what you've connected as context:

  • Information carries through where your template asks for it, for example the diagnosis list and medications from a previous letter

  • Dates stay accurate, so a previous appointment is described as, for example, "seen on 12 September" rather than as today

  • Today's consultation comes first, so findings from today aren't mixed up with previous ones

To check where a sentence came from, use Trace to… on it. If it came from a connected consultation, it'll point back to that consultation.

⚠️ Reporting issues: If you notice any hallucinations or omissions, or anything from a previous consultation that's presented as new, please let our support team know via the live chat button (bottom right of your Scribe window).
