Accurx Scribe: Template upload

Find out how to turn an existing letter or note template into Scribe template by uploading it, so you don't have to rebuild it from scratch in Template Builder.

Written by Helen Merriott

Many trusts already have well-established letter and note formats. Template upload lets you bring those straight into Scribe, so clinicians can keep documenting in the format your trust already uses.

📄 What is template upload?

Template upload lets you share an example of an existing document or note with Accurx Scribe. Scribe reads its structure and creates a draft Scribe template that matches it, including:

  • Headings from your original document

  • Free-text entries for sections written in prose (e.g. History of presenting complaint)

  • Structured entries for sections with fixed answers, such as select, multi-select or number fields (e.g. Smoking status: current / ex / never)

The draft then opens in Template Builder, where you can preview it, make any changes and publish it to your workspace. You can use template upload for both consultation notes and clinical documents, such as clinic letters.

👥 Who can use template upload?

Template upload is available to users who can create templates in Template Builder, usually workspace admins. If you can't see the option, speak to an admin in your workspace or get in touch with our support team.

✅ Before you start

You can upload any of these:

  • Word documents (.docx)

  • PDFs

  • Text or Markdown files (.txt, .md)

  • Images (.png, .jpeg)

  • Text pasted straight into the box

⚠️ Please only upload a blank or example template. Don't upload a real letter or note that contains patient information. If your only copy is a real document, remove any patient details first, or replace them with made-up placeholder text.

📌 How to create a template from an existing document

  1. Open Accurx Scribe from Accurx Web or Desktop.

  2. Click the settings option on the bottom left of your Scribe page, followed by 'Templates' 👇

  3. Select 'Create new'👇

  4. Choose 'Upload / Paste example' 👇

  5. Upload your file, or paste your template text into the box.

  6. Give Scribe a moment to read your document. It can take around 30 to 60 seconds to create your draft template.

  7. Your draft template will open in Template Builder, with headings and entries matching your original document.

✏️ Reviewing and publishing your template

Your uploaded template is a starting point, so it's important to check it before anyone uses it.

  1. Preview the template to check the layout matches your original document.

  2. Check each entry. Make sure sections with fixed answers have become structured entries, and prose sections are free-text.

  3. Make any changes you need. You can rename headings, add or remove entries, and reorder sections, just like any other Scribe template.

  4. Select the profession the template will mainly be used by, and give it a unique name.

  5. Publish the template to your workspace when you're happy with it.

Once it's published, you can share it with colleagues in the same way as any other Scribe template.

💡 Good to know

➡️ Always review your draft. Scribe does its best to match your original document, but it may not get everything right first time. Check every heading and entry before you publish.

➡️ Headings not covered in a consultation won't appear in the note. This is how all Scribe templates work. If a section isn't discussed, Scribe leaves it out rather than filling it with information that wasn't said.

➡️ Fine-tune with Memories. If Scribe isn't writing a section quite the way you'd like, you can add Memories to guide its style without changing the template structure.

➡️ Clinicians should still review every note. As with any Scribe template, notes and documents created with an uploaded template must be checked by the clinician before they're saved or sent.
