Capturing patient communication preferences is now easier than ever with custom questionnaires in Accurx.
If you use EMIS, you can create and send out the questionnaire below with linked SNOMED codes, and when a patient responds, their consent will be automatically recorded and seamlessly integrated into the patient record.
If you use SystmOne, once a patient responds to the below questionnaire, you can update their patient details in SystmOne.
The easiest way to send these messages is via Batch Messaging, but they can also be sent individually through Accurx.
Here's an example template for a custom questionnaire 👇
Step 1: Create patient questionnaire
Question: I understand that I am opting out of receiving communication via SMS and/or email from [Organisations/Service].
Description: You do NOT need to complete this questionnaire if you’re happy to receive communication via SMS and email.
Patients can select: Many Answers
Option 1
Answer: Yes, I want to opt out of SMS communication.
SNOMED code: Declined consent for SMS text messaging (Concept ID: 513631000000106)
Option 2
Answer: Yes, I want to opt out of email communication.
SNOMED code: Declined consent for communication by email (Concept ID: 835231000000104)
Step 2: Template Opt Out Message
Dear [patient name]
We are updating how we communicate with patients.
If you’re fine with receiving communication via SMS and email, you do NOT need to complete this questionnaire.
Please complete this questionnaire: [questionnaire link will be automatically generated]
Step 3: Post submission page message
Thank you for completing this questionnaire. We will update our records accordingly 👇
Step 4: Updating the medical record
If you’re using EMIS, there’s no need to take any extra steps—Accurx will automatically pick up the patient’s consent preferences from the SNOMED codes once the questionnaire is saved to the record.

If you’re using SystmOne, you can download the responses and use them to manually update the patient’s details, ensuring all communications align with their preferences. Accurx Desktop checks SMS consent status in the “Patient Details” section rather than using SNOMED codes.
