This article details what questions are asked in the coronavirus screening survey, and what you can save to the record.

What is COVID-19 triaging?

Given the presence of coronavirus in the UK, we've built a tool that helps practices triage patients who have booked face-to-face appointments before they come in. This can be used to screen patients due to visit the practice and help decrease the risk of potentially spreading the virus.

Once the patient has accessed the link, they will be prompted to enter their date of birth in order to access the questionnaire πŸ“†

Questions:

1. Do you have any of the following symptoms?

A new, continuous cough – this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours

A loss or change to your sense of smell or taste

Shortness of breath

Feeling tired or exhausted

An aching body

A headache

A sore throat

A blocked or runny nose

Loss of appetite

Diarrhoea

Feeling sick or being sick

  • Yes

  • No

2. Do you feel too unwell to do your normal activities (such as go to work).

  • Yes

  • No

3. Do you have a high temperature, fever, or shivering (chills)?

A high temperature means you feel hot to the touch on your chest or back ( you do not need to measure your temperature)

4. Have you tested positive for COVID-19 in the past 10 days?

  • Yes

  • No

(If β€˜Yes’ to question 4):

5. When did you test positive for COVID-19?

This is the date you took the test, which might not be the date you received your results

When they have submitted their answers they will be thanked for completing the survey and sent a confirmation SMS πŸ‘‡
