An Accurx questionnaire allows you to send a questionnaire out to patients to gather data. The waiting list validation questionnaire was built to help hospitals reduce their waiting lists by asking patients to confirm if they still need their appointments or procedures.
There are four waiting list validation questionnaires you can choose from:
Waiting List Validation - Appointment
The standard questionnaire that simply asks patients to answer with a yes/no whether they still need their appointment
Waiting List Validation - PIFU
For services putting their follow-up patients onto a Patient Initiated Follow-Up (PIFU) list
Waiting List Validation - Procedure
A questionnaire that asks patients to answer with a yes/no whether they still need their procedure
Waiting List Validation - Will Travel
For services that ask patients whether they are happy to travel to another provider for faster care
Enrolment SMS
Once the patient has accessed the link, they will be prompted to enter their date of birth in order to access the questionnaire π

When they have submitted their answers, they will be thanked for completing the survey and sent a confirmation SMS π
