Accurx Questionnaires allow you to quickly and easily gather structured information from patients. The Waiting List Validation Questionnaire helps services to reduce their waiting lists by asking patients to confirm if they still need their appointments or procedures.

Once the patient has accessed the link, they will be prompted to enter their date of birth to access the questionnaire πŸ“†

This questionnaire can either be sent individually or by batch. Please click here to find out how to send a batch questionnaire.

Questions (for Waiting List Validation Community and Mental Health):

  1. Do you still require this appointment?

We are required to regularly contact all patients awaiting care to see if their circumstances have changed while they have been waiting. This helps us keep our records up-to-date and ensures patients who need treatment are seen as soon as possible.

Please be aware that if you select "No" you may be discharged from the service and removed from the waiting list.

If you are unsure, please select "Yes" to remain on the waiting list.

  1. Yes, I still require an appointment

  2. No, I no longer require an appointment

If the patient selects No:

  1. Please tell us why you no longer require this appointment

  1. I have had care somewhere else

  2. My symptoms have resolved

  3. I do not wish to be seen anymore

  4. I do not want to give a reason

When they have submitted their answers, they will be thanked for completing the survey and sent a confirmation SMS πŸ‘‡

Monitoring your patient responses in the Accurx Inbox

Questionnaire responses will come into the Accurx inbox.

To view responses in the inbox:

  1. Open the Accurx and navigate to "Questionnaires" on the left hand side of the inbox

2. Within "Questionnaires", search for the "Waiting List Validation - Community and Mental Health" folder. Here you will then be able to see an individual's response

3. If not already saved to the record (i.e. the questionnaire was sent individually or this option wasn't selected when sent via batch), save the response to the record

Monitoring your patient responses sent via Batch

If you sent the questionnaire via batch, you can also download a csv with all the questionnaire responses

  1. Click on your initials in the Accurx toolbar

  2. Select "Batch Messaging" from the drop down

  3. Find the batch message you sent

  4. Click "View details" on the right hand side

  5. Under responses click "Download"

FAQ

Why are the responses not automatically saving to the record?

We felt that for more medically oriented questionnaires, such as the Waiting List Validation questionnaire, it is important that a user is notified of a response and that the response is viewed on the Accurx inbox before the decision to save it to the record is made.
