Accurx Scribe can now split a single consultation note into several separate notes, each reflecting a different problem discussed. This is especially useful for multi-problem consultations; for example, a hypertension review that also touches on low mood and diabetes history, where you'd want each issue documented as its own entry rather than one long note.
Note: This feature is currently available for users in Primary Care only.
How to split a note
Once your consultation has finished and your note has been generated, go to the Note tab and click the scissor icon next to it.
2. A "Split this note" window will open, showing how Scribe proposes to divide the note. The From section shows your original note title, and the To section lists the separate notes Scribe has identified (for example, "Hypertension," "Low mood," and "Diabetes history").
3. You're in full control of how the note is divided:
Edit the name of any proposed note by clicking into its text field and typing a new title.
Remove a split you don't want by clicking the bin icon next to it.
4. Once you're happy with the titles and structure, click Confirm.
Here's an example of the split reviewed and renamed before confirming: "Low mood" has been updated to "Low mood History" and "Diabetes history" to "Type 1 Diabetes":
5. After confirming, each problem appears as its own tab alongside Transcription (for example, Hypertension, Low mood History, and Type 1 Diabetes).
6. Click into any tab, such as Type 1 Diabetes, to see the full story for that problem unfold instantly. The note is already populated with the relevant history, comments, and details specific to that issue.
7. Each split note can be transferred to your medical record system individually. Save or copy each note into EMIS or SystmOne separately, as you would with a standard Scribe note.
8. If you want to remove one of the split notes, you can close it individually; this removes just that note, while the remaining split notes stay as they are.
⚠️ As with all Scribe outputs, it's important to review and edit each split note before saving it to the patient record.
