You can edit directly in the note by hovering your mouse pointer over the text field you want to change, and then editing the text directly.
Remove entry
If you want to remove the whole entry (a heading and its text) from the note, you can hover over the entry you want to remove and click the icon that looks like a trash can.
Add entry
If an entry is missing, you can choose to add one by clicking on "Add entry" at the bottom. A box will open where you can scroll to find the correct entry or search for it. Once you’ve selected an entry, it will automatically appear in the note, and you can fill it in.
Dictate directly into an entry
Click the microphone icon to dictate directly into any field within the note. When you’re finished, press the stop button to end the dictation.
Add and edit note
Tandem includes a built-in feature to automatically add more to your note or adjust your note based on your instructions using AI. Click the "Add or Adjust" button to open this feature.

You can give instructions on how you’d like Accurx Scribe to automatically modify the note. For example, you may want to add more information or rephrase sections, such as summarising the note or a specific entry.
How to use:
1. Press the 'Add or adjust' button
At the bottom of the note view, select "Add or adjust."
2. Provide instructions to Accurx Scribe
Accurx Scribe starts recording automatically, so you can immediately instruct the Scribe on what you want to add or change in the note. If you prefer to give written instructions, that works too—simply type what you want to add or change in the text box.
For example:
"Under the heading Assessment, add the following..."
"Remove everything that mentions... throughout the entire note."
"Add all details about the patient's chest pain under the heading History."
"The neurological examination was normal."
You can pause and resume voice recording by clicking the red record button.
Then click the symbol to let Accurx Scribe execute your instructions and automatically update your note.
3. Review and undo adjustments
Fields adjusted by Accurx Scribe will be highlighted with a blue background. Review the changes and edit if needed. If you want to revert all adjustments Accurx Scribe has made in a particular section, press "Reset" to restore the text to its original state.
Accurx Scribe can also use text from sources like previous medical records to generate a new, structured note or document. This is done using the “Add additional text” button, which appears during the recording.
You can use this to add relevant information—such as earlier notes or a digital intake completed before the visit. Tandem combines this input with the recording to create the final note. You can also generate a note based solely on the added text, for instance, if the patient interaction took place via chat.

In addition to regular notes, you can also create other documents in Accurx Scribe, such as patient letters, referrals, referral templates, and sick leave forms.
Here's how to do it:
Start and end your patient conversation or dictation as usual.
In the note view, click "Create other documents" at the top, then select the type of document you want Accurx Scribe to generate.

You can always pause an ongoing conversation and return to it later. For example, if your patient needs to go for a blood test and you want to see another patient in the meantime. You can find your paused conversations in the side menu.
You can also edit the title of a conversation during recording to easily keep track of multiple ongoing conversations.

Dot phrases allow you to add words or sentences that you frequently use in your medical notes and have quick access to them in Accurx Scribe.
Add phrases
Go to Settings > Phrase Library.
1. Click the "Add New Phrase" button.
2. Write or copy a name for your phrase, and then specify what the phrase should be. This will be used by Accurx Scribe when you insert the phrase into the medical note.
3. Click the "Add Phrase" button.
ⓘ You can add as many phrases as you want, and all the phrases you enter are personal to your login
Using phrases
1. Place your cursor in a text field in your medical note.
2. Type a semicolon ( ; ). A box will appear where you can search or browse your entered phrases..
3. Search for your phrase. You can either search by the phrase name or by any content within the phrase you entered. You can navigate through the search results using the ↑ (up) and ↓ (down) arrows on your keyboard, or by using the tab button.
4. Press the [enter] key to insert the selected phrase into the chosen text field.

For each phrase, there is a button with three dots ( ...). Press it to choose whether to edit the phrase or remove it from Tandem.

You can also use Accurx Scribe to dictate directly into a blank template after your consultation, instead of having a patient conversation that Accurx Scribe records.
Here's how to use it:
1. Activate Smart Dictation
You can activate Smart Dictation mode by adjusting your visit settings on the home screen or by clicking the down arrow (↓) at the top right.
On the home screen:

2. Dictate to Accurx Scribe
The template you selected will appear with all fields empty, and the recording will start automatically. Simply dictate what you want Accurx Scribe to place under each heading, then press the icon.
Accurx Scribe will then automatically insert what you’ve dictated under the appropriate heading.
For best results when dictating, use the format "Heading + Dictated text," and Accurx Scribe will place your dictation in the correct section.
For example:
"Subjective: Patient reports feeling fatigued and experiencing headaches."
"Objective: Blood pressure 130/85, heart rate 78 bpm."
"Assessment: Likely tension headaches."
"Plan: Recommend hydration, over-the-counter pain relievers, and a follow-up in one week."
Start and pause the recording of your dictation by clicking the microphone icon or the red stop button:
⚠️ Please review all notes and letters that you generate before saving them to record or copying them elsewhere.
