We have built a tool that helps healthcare teams remotely monitor their patients who may have been discharged from the hospital or who they may want to keep a closer eye on.

Once the patient has accessed the link, they will be prompted to enter their date of birth in order to access the questionnaire πŸ“†

1. How do you feel compared to yesterday?

3. Do you have any new symptoms that you are worried about?

4. Please select if you have any of the following equipment at home and are able to provide readings.

(Select all that apply)

  • Blood pressure monitor

  • Fingertip oxygen monitor

  • Thermometer

  • I have none of these

The patient will be prompted to answer a number of questions depending on the equipment they have available to them expr.

Blood pressure monitor questions

Questions 5 to 7 only show if the patient selects 'Blood pressure monitor' in question 4.

5. Please enter your systolic (SYS) blood pressure reading. This is the top number.

Please Note ⚠️ If a patient answers all the questions, we will collect both pulse readings from the blood pressure monitor and fingertip oxygen monitor questions. Both readings will be displayed in the inbox but the blood pressure pulse reading takes priority in being coded to the record.
