What is a BMI Questionnaire?

An Accurx questionnaire allows you to gather data about your patients. The BMI Questionnaire is used to help gather data on a patient's weight and height. The information will be coded along with a BMI.

This article details what questions are asked and which SNOMED codes are added.

💬 Enrolment SMS:

text message that will display to a patient receiving the BMI Questionnaire.

Once the patient has accessed the link, they will be prompted to enter their date of birth in order to access the questionnaire 📆

Questions:

1. Please enter your weight in kilograms.

(If you only know your weight in stone and pounds, please use the converter here)

  • Enter the number in kg

2. Please enter your height in metres.

(If you only know your height in feet and inches, please use the converter here)

  • Enter the number in metres

When they have submitted their answers they will be thanked for completing the survey and sent a confirmation SMS 👇

Response in the Accurx Desktop toolbar: BMI Questionnaire Folder 👇

Response displayed within the Accurx inbox.

Please then select the save to record button if you wish for these results to be saved to the patient's medical record 📚
