Accurx Web: How to send a questionnaire to a patient

Learn how to collect structured information from patients via SMS

What are Questionnaires?

These are mobile-friendly pre-made questionnaires for patients to collect data through their responses. The responses are then fed back to you via the Accurx Web Inbox.

You can then download these responses as a PDF to save them to your paper or electronic medical record. (See a screenshot of this below 👇)

The example we have used for this particular step-by-step guide is the Single Skin Lesion Questionnaire. However, the other Questionnaires work like this too.

Sending a Questionnaire to a patient

Step 1:

Search for a patient using their NHS number and date of birth (or a test patient if you want to give it a try for yourself).

Step 2:

Send a message to a patient by clicking the Message Patient icon on the top right of the patient profile

Step 3:

To send a questionnaire, select the 'See Questionnaires' option on the top right of the compose message screen as shown in the screenshot below 👇

Step 4:

Here you can see the available questionnaires.

Click the Questionnaire you’d like to send to the patient, e.g. “Single Skin Lesion Questionnaire” by selecting “Use questionnaire”, and the text message will pre-fill (but feel free to edit it to make it more personalised!).

Click the “Send” button and the message will be on its way to the patient!

Reminder SMS Messages

Patients will receive two reminder texts on day 3 and day 6 if they have not completed the survey within 7 days.

Processing individual patient questionnaire responses 👱

Step 1:

When a new response comes back from a patient, you will see a notification on the Conversations tab in Accurx Web.

Step 2:

Once you open the Conversations tab, you should navigate to the “Questionnaires” section and click on the questionnaire you want to view responses for (e.g. “Single Skin Lesion”) as shown below.

Step 3:

When you click on the “Single Skin Lesion” inbox, you will see all patients who responded to the questionnaire on the right-hand pane.

When you select one of the patients, you can review their response. You will also be able to get a copy of the patient’s response that you can then add to the medical notes, by selecting “Download as PDF”.

Processing multiple patient questionnaire responses 👪

To download a CSV file of all the patient responses for the batch message please follow these steps. 👇

Step 1:

Click the "Batch Message" button on the left-hand side of the screen.

Step 2:

Select "Sent messages" in the menu that appears.

Step 3:

Find the Batch Questionnaire you'd like to view the results of and select the "View details" button on the right-hand side.

Step 4:

In the details page of the Batch Questionnaire, click "Download" under the responses header. This will download a CSV file of the patient responses to each question in the questionnaire, as well as additional details such as the time the message was sent and when the patient responded to the questionnaire.
