A guide on our waiting list validation questionnaires

A Florey allows you to send a questionnaire out to patients to gather data. The waiting list validation questionnaire was built to help hospitals reduce their waiting lists by asking patients to confirm if they still need their appointments or procedures.

There are four waiting list validation Florey questionnaires you can choose from:

  • Waiting List Validation - Appointment

    • The standard questionnaire that simply asks patients to answer with a yes/no whether they still need their appointment

  • Waiting List Validation - PIFU

    • For services putting their follow-up patients onto a Patient Initiated Follow-Up (PIFU) list

  • Waiting List Validation - Procedure

    • A questionnaire that asks patients to answer with a yes/no whether they still need their procedure

  • Waiting List Validation - Will Travel

    • For services that ask patients whether they are happy to travel to another provider for faster care

Enrolment SMS

Copy of the SMS patients will receive regarding wait list validation.

Once the patient has accessed the link, they will be prompted to enter their date of birth in order to access the questionnaire πŸ“†

Questions (for Waiting List Validation - Appointment):

  1. Do you still require this appointment?
    ​We are required to regularly contact all patients awaiting care to see if their circumstances have changed whilst they have been waiting.
    ​
    This allows us to keep our records up-to-date and to ensure that patients who need treatment are seen as soon as possible

    1. Yes

    2. No

If the patient selects No:

  1. Please tell us why you no longer require this appointment
    ​This helps us to ensure our records are accurate

    1. I have had care somewhere else

    2. My symptoms have resolved

    3. I do not wish to be seen anymore

    4. I have already had my appointment at the hospital

    5. I do not want to give a reason

When they have submitted their answers, they will be thanked for completing the survey and sent a confirmation SMS πŸ‘‡
