What is an Ethnicity Questionnaire?
An Accurx questionnaire allows you to send a questionnaire out to patients to gather data.
The Ethnicity questionnaire is used to help gather data on the Ethnicity of a patient. The response can be saved back as a coded entry (using the 2011 Census codes).
Please note: This questionnaire is only available to users who are in an area that has purchased Accurx Plus.
