What is a smoking status Florey?

A Florey allows you to send a questionnaire out to patients to gather data. The Smoking Questionnaire Florey is used to help gather data on a patient's smoking status.

This article details what questions are asked and which SNOMED codes are added, according to the patient's responses.

Once the patient has accessed the link, they will be prompted to enter their date of birth in order to access the questionnaire πŸ“†

Questions:

1. What is your smoking status?

  • Current smoker

  • Ex-smoker

  • Never smoked (questionnaire ends)

(If selected "current smoker")

2. How much do you smoke?

  • <1 cigarettes or gram/day or equivalent

  • 1-9 cigarettes or gram/day or equivalent

  • 10-19 cigarettes or gram/day or equivalent

  • 20-30 cigarettes or gram/day or equivalent

  • 40+ cigarettes or gram/day or equivalent

(If selected "ex-smoker")

2. How much did you used to smoke?

  • <1 cigarettes or gram/day or equivalent

  • 1-9 cigarettes or gram/day or equivalent

  • 10-19 cigarettes or gram/day or equivalent

  • 20-30 cigarettes or gram/day or equivalent

  • 40+ cigarettes or gram/day or equivalent

(If selected "ex-smoker")

3. When did you stop smoking?

  • Less than a year ago

  • More than a year ago

When they have submitted their answers they will be thanked for completing the survey and sent a confirmation SMS πŸ‘‡
