Questionnaire: Oxford Hip Score (OHS)

We have built a tool that helps healthcare teams gather patient-reported outcomes to assess function and pain with patients undergoing hip replacement surgery.

Questions:

1. On which side of your body is the affected hip for which you are receiving treatment or experiencing symptoms?

(If you say ‘both’, please complete this first questionnaire thinking about the right side. A second questionnaire, for the left side, will follow.)
