What does this Questionnaire do?
The Accurx Adult ADHD Self-Report Scale (ASRS) Questionnaire allows patients over 18 to answer questions about potential symptoms of Attention deficit hyperactivity disorder (ADHD).
The questionnaire link is sent via SMS or email. The patient fills the questionnaire in through a browser so they can use their phone, tablet, laptop or desktop to complete it.
The responses are returned to the Inbox and can be saved directly into the clinical record (EMIS, SystmOne or Vision). The responses can have additional notes added, be assigned to other colleagues and be marked as urgent.
The questionnaire obtains answers from the patient and then provides the clinician with the raw information along with a score.
How is it scored?
The score generated depends on the answers to the first 6 questions (Part A). A mark is given if any answer falls in the grey-shaded boxes of part A (see above). Four or more marks, as per the adult ASRS, may warrant further investigation into adult ADHD.
The frequency scores in the last 12 questions (Part B) don't form part of the score, but provide additional cues and can serve as further probes into the patient's symptoms.

The patient continues to answer the questions as per the Adult ADHD Self-Report Scale (ASRS) Symptom Checklist Questionnaire until all 18 have been answered.
The patient is then presented with a summary screen to confirm they are happy with the answers.
