The 4d BP Home Monitoring Questionnaire allows patients to submit two blood pressure readings remotely twice daily over 4 days. The patient will receive daily SMS reminders.

Please note: This questionnaire is only available to users who are in an area that has purchased Accurx Plus.

Intro before questions

Before you submit your blood pressure reading, here are some tips to get the most accurate result:

Before you take your blood pressure reading:

  • Sit down comfortably for 5 minutes.

  • Wear loose-fitting clothing.

  • Make sure your arm is around the same level as your heart. You may need to support your arm with a cushion.

  • Make sure your arm is relaxed, not tense.

When taking your blood pressure:

  • Put the cuff on following the instructions which came with your blood pressure monitor.

  • Keep still and silent. Moving and talking can affect your reading.

Other tips:

  • Take at least two readings, each two minutes apart. Some people find that their first reading is much higher than the next readings. If this is true for you, keep taking readings until they level out and stop falling, then use this as your reading.
