Florey allows you to send questionnaires out to patients to gather data. The Anxiety GAD-7 Florey Questionnaire is used to help gather data that would be useful for an anxiety review. Using the information gathered, clinicians can determine the most helpful subsequent steps in the review e.g. video call.

Answering the following 7 questions will provide us information on your anxiety. Please try and answer as honestly as your can.

The responses are usually reviewed by your practice within 7 days but if you feel too unwell to wait for a response, please call them (or ring 111 if your practice is closed).

1. Over the last 2 weeks, how often have been bothered by feeling nervous, anxious, or on edge?

2. Over the last 2 weeks, how often have you been bothered by not being able to stop or control worrying?

3. Over the last 2 weeks, how often have you been bothered by worrying too much about different things?

4. Over the last 2 weeks, how often have you been bothered by having trouble relaxing?

5. Over the last 2 weeks, how often have you been bothered by being so restless that it’s hard to sit still?

6. Over the last 2 weeks, how often have you been bothered by becoming easily annoyed or irritable?

7. Over the last 2 weeks, how often have you been bothered by feeling afraid as if something awful might happen?

When they have submitted their answers they will be thanked for completing the survey and sent a confirmation SMS 👇
