What is the Alcohol Health (AUDIT-C) Questionnaire?
The Alcohol Health Questionnaire is based on the standard AUDIT-C questions. This article details what the patient will see in the text, and what gets written to the record for their responses.

2. How often do you have a drink containing alcohol?

3. How many units containing alcohol do you drink on a typical day when you were drinking?
(1 pint of beer is approximately two units, and one small glass of wine is 1 unit)

7 to 9 units
10 or more units
(When saved back to record - "Units of alcohol drunk on a typical day: 3-4 units")
4. How often did you have 6 or more units if female, or 8 or more if male, on a single occasion in the past year?

When they have submitted their answers they will be thanked for completing the survey and sent a confirmation SMS π
