What does Accurx Scribe do?

Accurx Scribe, powered by Tandem, uses AI to transcribe audio from a patient consultation, dictation or meeting, and to summarise it into a structured form for medical notes. Clinicians can also generate other documents from the encounter, such as patient letters or referral letters. After reviewing and editing these, clinicians can save their notes to the patient record and share documents with patients and other services.

Accurx Scribe is available via Accurx Desktop, Accurx Web and Accurx Mobile.

Can Accurx Scribe make mistakes?

Accurx Scribe is powered by Tandem's AI. Whilst it's highly accurate, it can make mistakes, such as missing important information out (omissions) and confabulating incorrect information (hallucinations).

It's very important that the responsible clinician reviews, and edits if necessary, all the Accurx Scribe outputs before adding them to the medical record or sharing documents with patients or other services.

Does Accurx Scribe comply with relevant clinical safety standards?

Yes. Accurx Scribe, powered by Tandem, is compliant with DCB0129, the NHS standard for Clinical Risk Management: its Application in the Manufacture of Health IT Systems. Is Accurx Scribe a medical device?

What steps has Accurx taken to reduce the clinical risks of using AI scribe technology?

Accurx has taken a layered approach to clinical safety, with each layer reducing the risks in different ways. More details below.

Tandem AI

Accurx is powered by Tandem's AI, which makes use of industry leading transcription and summarisation models. In addition, Tandem has built additional controls on top of these models to reduce the likelihood of them producing hallucinations and to improve how the scribe handles medical terminology.

Importantly, Tandem's AI is prompted not to suggest a diagnosis, management or treatment plan.

Ensuring the clinician is in-the-loop

Accurx Scribe outputs are highly accurate, but they can include errors. It is essential that the responsible clinician reviews, and edits if necessary, all the Scribe outputs before adding them to the medical record or sharing documents with patients or other services.

Accurx Scribe includes measures to help ensure the clinician always reviews their Scribe outputs. These include an in-product tutorial on how to use the product safely, a permanent warning advising clinicians to review Scribe outputs, and a pop-up warning reminding clinicians to review their notes if they try to save them or copy them without editing them.

Training and support

Accurx provides a range of training and support materials to help NHS staff use Accurx Scribe safely and effectively. These include support articles (like this one!), live training sessions that can be booked here, and on-demand resources here. Accurx also offer live chat support via the text bubble icon at the bottom right of this screen.

Microphone quality

For the best quality transcriptions Accurx recommend you use a high-quality microphone. When you first use Accurx Scribe you will be taken through a tutorial that includes checking your microphone is working.

If you do not have a high-quality microphone attached to your computer, you can use Accurx Scribe on your smartphone via the Accurx Mobile app. This enables you to make use of the high-quality microphones in your smartphone. Your consultations will then be immediately available for you to review on your computer via Accurx Desktop or Accurx Web.

Post-market surveillance, incident management and testing

Tandem are conducting regular audits to monitor for the safety and accuracy of Scribe outputs. Users of Accurx Scribe are also able to submit any safety concerns via the in-product feedback tool in Accurx Scribe or via Accurx Support (support@accurx.com).

If any clinical issues are identified with Accurx Scribe they will be handled according to Accurx's well established incident management processes.

Whenever Tandem make changes to how its AI works they test those changes in a variety of ways, including regression testing, to check that the model performance has improved.
