We're Accurx: a software company that healthcare providers and patients use to contact each other. We're being used by patients to contact their GP practice about a health problem.
π’ We are currently piloting a new feature in some practices for when patients submit a request to their GP. If there are a few services that might be able to help you with the health problem you have described, you will see some information about these services and how you can access them. If your practice doesn't have this feature, please see this article which shows you how to submit an online request.
How to use the Patient Triage form to contact your GP about a health problem
Please note! If your medical query is urgent then contact β111β or for a medical emergency contact, β999β.
If you want to submit a query about a health problem to your GP practice please visit their website. Here you should find a link that allows you to begin submitting your query. However, if this is not clear then you will need to contact your GP practice.
Once you have clicked on the link the form will open. At the top of the screen you will see the details of the practice - please check this is the GP practice you are registered at. If you want to submit a query about a health problem please select this option π
Next you will be prompted to select if the query is for yourself or someone else, then you can put the patients date of birth in.
Note: if you are accessing this form through the NHS app or NHS log in, the date of birth will be filled automatically.
Next you will be asked to confirm whether you are experiencing any of the βred flagβ medical symptoms. π The symptoms you see on this page will vary depending on the patients age.
If you are experiencing any of these symptoms, do not continue with the form; call 999.
If you aren't experiencing any of these symptoms and you have confirmed this, you can now describe the health problem. π
If several services might be able to help you with the health problem you have described, you will see some information about these services and how you can access them. Depending on the symptoms or condition described, there's a section where you can check if you need more urgent help.
For example, if you are looking for help with a sore throat, you will be given information on how to find a pharmacy that can offer treatment and advice. This page also signposts to NHS online advice.
This page will show information on different services depending on the symptoms or health condition described. π

I still want to contact my GP
If these options aren't suitable and you still want to contact your GP, you can do this by clicking the 'Contact GP button.
This will take you to a page where you can add more information about the problem you are experiencing π
Once you complete the form and submit it to your GP, make a note of your reference number in case they need to reach out about your request. π
Why am I seeing information about other services?
There are several services that can provide advice and care for certain conditions that patients can access themselves without needing to see a GP. Using these services doesn't require a GP appointment and may be easier and quicker to access.
