Who is Accurx?
We are a technology company that provides communications software to healthcare providers across the NHS.
Our online consultation solution, called Patient Triage, is recognized by the NHS and has passed the NHS Digital Capabilities assessment. See the link here for more information.
The online consultation form
The Patient Triage solution allows patients to submit either a medical query or an administrative request to their registered GP practice.
Please note! If your medical query is urgent then contact β111β or for a medical emergency contact, β999β.
If you want to submit a query to your GP practice you will need to visit their website. A link should be provided that allows you to begin submitting your query. However, if this is not clear then you will need to contact your GP practice.
Confirmation of submission
Once you click the Submit request button, you will be navigated to the next page confirming that your request form has been submitted.

Reference ID
Instructions on how to use the reference ID
What is the Reference ID?
You will see a reference ID made up of a mixture of letters and numbers once you have submitted your request. π
This reference ID is proof that your request has been submitted to the practice listed on the top left corner of the page.
You can use the 'Copy' button to make a note of your reference ID somewhere.
When do I need to use the Reference ID?
Once the request has been submitted, it will be delivered to the practice's inbox. They may take a few days to review the request and respond to you.
Please wait patiently until they respond and contact the practice directly if you have not heard back after the time frame shown on the submission page.
If the practice is unable to find your request, they may ask you for your reference ID as proof that you submitted the request. They are not able to use this to search within the inbox so they will reach out to Accurx so we can provide the information for them.
Only share your reference ID with your practice to help them find your request.
